An excellent aid in the quest for complication free surgery, and just in time, CRST and this year’s annual complications issue. I particularly liked the multiple articles on complication prevention and avoidance, always the best strategy for complication management. Jai G. Parekh, MD, MBA gives an excellent summary of tips to help surgery go more smoothly. Daniel H. Chang, MD has a detailed perspective on Purkinje images to help better center IOLs to avoid dysphotopsias, especially useful for multifocal IOLs. Christopher E. Starr, MD gives us a detailed description on a new phaco technique designed to minimize risk that he has used in training residents. William F. Wiley MD writes on IOL calculations and how to avoid errors in lens power selection. Richard Tipperman, MD gives his thoughts on the proper use of a piggyback IOL to avoid complications. This article is expanded as Dr. Tipperman joins Mark Packer MD in a joint article with even more detail on the technique. I found Richard Packard, MD, FRCS, FRCOPHTH’s article especially useful as a review of the variety of possible complications with the IOL itself after a successful cataract removal.
I’ll leave with another pearl from Lee. He told me the way to recognize a good surgeon is not necessarily by watching him or her doing routine cataracts out of a long list of routine cases but rather watching the surgeon handle a complication. You truly see the skill when a surgeon has to go outside of his routine, shift gears, and improvise. Lee was right, and I hope you enjoy these articles as much as I did.
